PLAYER PARTICIPATION WAIVER OF LIABILITY

WHEREAS,(player's name) _______________________________ (hereinafter referred to as ("I" or "my") desire to participate as a Flag Football participant and International Women's Flag-Football Association (hereinafter referred to as "IWFFA"); and

WHEREAS, I desire to play for the International  Tournament, clinic or event known as (your team) _____________________________ during year 2006; and

WHEREAS, I desire to participate voluntarily for my own personal enjoyment; and

WHEREAS, the IWFFA wishes to permit me to participate in the International Tournament provided I agree to waive and release any claims for any personal injury, death and property damage which I may sustain as a result of my participation in the event as set forth below.

NOW, THEREFORE, in consideration of the above and in consideration for the IWFFA granting me permission to participate in the TOURNAMENT/ clinic or event, I acknowledge and agree to the following:

1. The IWFFA is defined to include, but is not limited to, the association itself, any and all organizers, officials, officers, directors, team sponsors, coaches, and any other individual or entity having any control, responsibility or authority over the matters and functions of the association.

2. "Participation" or "participate" is defined to include, but is not limited to, playing in IWFFA games, clinics, trainings participating in team practices, and any other function(s) of the team or IWFF

3. Upon signing this writing, I realize that I cannot play for any other team in the IWFFA Tournament, other than the team designated above.

4. I am aware of the possible risks, hazards and dangers normally associated with playing flag football and I voluntarily assume the risk of all such risks, hazards and dangers including, but not limited to, physical injury, death and property damage. I also assume the risk of all dangerous conditions in and about the playing and practice fields and dangerous weather conditions. I will visually inspect each field prior to each practice or game and will rely upon my own inspection in determining whether or not to participate. Participation on my part is voluntary and I have the absolute discretion to refuse to play, practice or participate as the case may be.

5. I have physically trained for my participation as a flag football player in the IWFFA  International Tournaments, clinics and related events and I am unaware of any physical or health condition which would prevent or limit my participation in the tournament.

6. RELEASE/WAIVER: I HEREBY WAIVE AND RELEASE AND AGREE TO HOLD HARMLESS THE IWFFA FROM ANY AND ALL CLAIMS, DAMAGES, CAUSES OF ACTION, SUITS AND LIABILITY, OF ANY KIND FOR ANY PERSONAL INJURY, DEATH OR PROPERTY DAMAGE WHICH I MAY SUSTAIN ARISING OUT OF OR OCCASIONED BY MY PARTICIPATION IN THE IWFFA-HOUSTON TOURNAMENT. THIS WAIVE AND RELEASE SHALL ALSO BE BINDING UPON MY SPOUSE, PARTNER, HEIRS, PERSONAL REPRESENTATIVES AND ANY OTHER PERSON WHO MAY CLAIM THROUGH ME.
